The influence of bias- ommissions

Patient-centred consulting doesn't mean doing what the patient wants.

It's about putting the patient's needs at the centre of the discussion.

The GP would justify his approach as upholding the autonomy of an informed and engaged patient in their health decisions, yet this embracing of the request results in skimping on the information the patient needs, to address his concerns about Prostate Biopsy at least, alongside the potential other concerns he may not yet be aware of, as seen in the other versions of this case.

It's clear from his non-verbal cues that he's expecting more from this short consultation, to explore a little more about the risks.

Remember the Choosing Wisely questions: (BRAN)

  • Benefits of the proposed intervention or treatment,
  • Risks of the proposed intervention or treatment,
  • Alternative choices available,
  • Nothing option - what would happen if nothing were done

The persons agenda health beliefs and or preferences were insufficiently explored

The examiners felt the consultation was insufficiently person-centred.

It is important to understand the context of the illness; and attach importance to the subjective aspects of medicine. This means always seeing the patient as a unique person and being curious; then valuing patient preferences and expectations.

For example, a patient whose neighbour has lung cancer may have fears about their own cough. Another patient may be vaccine hesitant due to health beliefs which are understandable and strongly held in their community.

Suggestion

Ensure you explore the person's ideas, concerns and expectations; also consider the impact the situation has had on their life.
